Surgical Technique Brings Lasting Relief for Rare Form of Hyperacusis

A new report published in Otolology & Neurotology has described successful long-term results from a delicate ear surgery that relieved severe sound sensitivity (hyperacusis) in patients with a rare inner ear condition known as Superior Semicircular Canal Dehiscence (SSCD) syndrome.

The study, led by Mr. Anirvan Banerjee and Dr. Chon Meng Lam, followed three patients who underwent round window reinforcement (RWR) surgery as a secondary treatment after initial SSCD repair. All three patients experienced a complete resolution of hyperacusis symptoms in the treated ear, with benefits lasting up to eight years.

What is SSCD and why does it cause sound sensitivity?

SSCD occurs when a small hole or thinning develops in one of the inner ear balance canals. This can lead to dizziness, a sense of pressure, and extreme sensitivity to sound, where even normal noises feel painfully loud. In some cases, sound can even cause vertigo or imbalance.

A second surgery for persistent hyperacusis

In the reported cases, patients had previously undergone surgery to seal the dehiscence but continued to experience hyperacusis in one ear. The second operation, called round window reinforcement, involves carefully strengthening a thin membrane in the middle ear to stabilise sound transmission.

Encouraging long-term outcomes

After the procedure, all three patients reported complete relief from hyperacusis in the operated ear. Follow-up periods ranged from several years to almost a decade.
While two patients experienced a mild degree of hearing loss, the overall outcome was positive, and no recurrence of sound sensitivity occurred on the treated side.

Dr Hashir Aazh, Director of the Hashir International Institute, commented:

“These findings are very encouraging for people with severe hyperacusis linked to SSCD. Although this condition is rare, it can be devastating. Surgical reinforcement of the round window offers a potential option for carefully selected patients when standard approaches do not provide relief.”

Careful assessment remains essential

The authors emphasised the importance of patient selection and detailed counselling before surgery, as there is a small risk of hearing changes.
